I give permission to the Aboriginal Health and Medical Research Council of New South Wales (ABN 66 085 654 397), of Level 4, 280 Pitt Street, Sydney NSW 2000, and its assignees, successors, licensees, legal representatives, employees, and agents (“AH&MRC”), the irrevocable right to use my name, photograph, image, audio recording/video recording and likeness, or if applicable my child’s name, photograph, image, audio recording/video recording and likeness (“My Image and/or if applicable My Child’s Image.”), in any form and manner.
This permission includes use for advertising, media publicity, publication, general display, or any other purpose, whether in whole or in part, including but not limited to use on internet websites, broadcasts, and other publications (“the Publication”).
I waive any interest I may have now or in the future in the copyright of My Image and/or if applicable My Child’s Image. I acknowledge that neither I, nor my child, are entitled to receive, now or in the future, any payment or other consideration for the use or publication of My Image and/or if applicable My Child’s Image. I agree not to make any claim against AH&MRC for payment relating to the Publication of My Image and/or if applicable My Child’s Image. I understand that AH&MRC cannot control any unauthorised use of My Image and/or if applicable My Child’s Image by persons not associated with AH&MRC once the Publication has occurred.
I irrevocably waive any right to inspect or approve the Publication of My Image and/or if applicable My Child’s Image by AH&MRC. I release and indemnify AH&MRC from any loss, damage, cost, expense, or claim (including consequential loss) arising from or connected with the Publication of My Image, including but not limited to claims for defamation, libel, breach of privacy, or copyright.
